This 47 year old lady had carpal tunnel surgery 2 year prior and had excellent symptoms relief for 6 months after which she noted scar tenderness which was aggravated by contact. This has led her to avoiding exposing the scar and making work as a cashier difficult.
On examination the scar was exquisitely tender to touch and had a positive Tinel’s along the median but Phalens was negative. She has no residual numbness in the median nerve distribution. This is the findings on exploration of the carpal tunnel.

What is the clincial problem here?
